Insights
Intravenous posaconazole dosing in children requires higher doses per body weight than adults to achieve therapeutic antifungal concentrations. Adverse effects were minimal in a small pediatric study.
Area of Science:
- Pharmacology
- Pediatric Medicine
- Infectious Diseases
Background:
- Posaconazole is a broad-spectrum antifungal agent.
- Intravenous (IV) formulation is used for invasive fungal infections.
- Limited data exists on pediatric dosing for IV posaconazole.
Observation:
- Four pediatric patients (3-9 years) received IV posaconazole.
- Treatment was for documented or suspected invasive fungal infections.
- Dosing ranged from 8.4 to 12.2 mg/kg daily.
Findings:
- Pediatric patients achieved therapeutic serum concentrations of posaconazole.
- Doses required were higher per body weight compared to adult recommendations.
- Minimal adverse effects were observed during treatment.
Implications:
- Higher IV posaconazole doses may be necessary for pediatric patients.
- This study provides initial data for pediatric dosing guidelines.
- Further research is warranted to establish optimal pediatric dosing strategies.
Background:
Posaconazole is a broad-spectrum antifungal used for prophylaxis and treatment of invasive fungal infections. There is no published data to inform prescribers on dosing of the intravenous (IV) formulation in the pediatric population. We describe our experience including dosing, serum concentrations, and tolerability.
Observations:
Four patients (3 to 9 y) received IV posaconazole for treatment of documented/suspected invasive fungal infections. Patients achieved therapeutic concentrations on daily doses of 8.4 to 12.2 mg/kg and adverse effects were minimal.
Conclusions:
Higher dosing per body weight of IV posaconazole may be required in the pediatric population compared with adults to consistently achieve therapeutic concentrations.
