Revascularization for Aplastic or Twiglike Middle Cerebral Artery: A Case Report

Aplastic or twiglike middle cerebral artery (Ap/T-MCA) is a rare congenital anomaly. Revascularization surgery successfully treated a patient with Ap/T-MCA, preventing further strokes.

Background:
- Aplastic or twiglike middle cerebral artery (Ap/T-MCA) is a congenital anomaly.
- It results from abnormal embryonic development of the middle cerebral artery (MCA).
- Ap/T-MCA can lead to stroke due to hypoperfusion and fragile collateral circulation.
Observation:
- A 19-year-old woman presented with transient ischemic attack symptoms including hemiparesis and headache.
- Digital subtraction angiography revealed MCA M1 segment occlusion and abnormal distal vascular networks.
- The patient experienced transient left upper limb hemiparesis, dysesthesia, and headache.
Findings:
- The patient underwent successful direct and indirect revascularization (superficial temporal artery-to-MCA bypass and encephalomyosynangiosis).
- Post-surgery, the patient had no neurological deficits.
- Follow-up angiography showed regression of abnormal vascular networks.
Implications:
- Revascularization may be a viable treatment for Ap/T-MCA.
- This procedure can reduce microvascular collateral stress.
- It may help prevent future strokes in carefully selected patients with this anomaly.
