    Background:

    • Ileal neobladders require a balance between reservoir capacity and surface area to minimize urine metabolite resorption and metabolic disturbances.
    • Standard neobladders, created from longer ileal segments, can lead to systemic metabolic complications.
    • A too-small pouch may result in urinary frequency and incontinence.

    Purpose of the Study:

    • To investigate the feasibility of creating a neobladder from a shorter ileal segment.
    • To assess if a shorter segment reduces metabolic complications compared to standard techniques.

    Main Methods:

    • A prospective study comparing 77 male patients undergoing radical cystectomy and orthotopic neobladder.
    • Patients were divided into a standard pouch (SP) group (50–70 cm ileal segment) and a Belgrade pouch (BP) group (25–35 cm ileal segment).
    • Neobladder capacity, acidosis, base excess, and bicarbonate levels were measured postoperatively.

    Main Results:

    • The SP group had significantly higher neobladder capacity (mean 750 mL) than the BP group (mean 438 mL) (p < 0.001).
    • The SP group showed higher rates of acidosis (37.8% vs. 2.5%), base excess (35.1% vs. 7.5%), and low bicarbonate (40.5% vs. 20.0%) compared to the BP group (p < 0.001).

    Conclusions:

    • The Belgrade pouch, constructed from a 25–35 cm ileal segment, achieves adequate capacity.
    • This technique results in a lower incidence of metabolic disturbances compared to standard, high-capacity orthotopic neobladders.
