    Background:

    • Diagnosing descending aortic abnormalities retrocardiacly via echocardiography is challenging.
    • Conventional echocardiography has limitations in visualizing the retrocardiac descending thoracic aorta.

    Observation:

    • A novel "paravertebral approach" using cross-sectional echocardiography was prospectively evaluated.
    • Two patients with dissecting aneurysms underwent imaging in the right lateral position with a specialized transducer placement.
    • This approach allowed visualization of the retrocardiac descending aorta in both long and short axes.

    Findings:

    • The paravertebral approach clearly visualized the descending thoracic aorta and oscillating intimal flaps.
    • Pulsed Doppler echocardiography revealed distinct flow patterns in true and false lumens, with higher velocities in the true lumen.
