    Background:

    • Alcoholic patients are prone to severe lactic acidosis.
    • Lactic acidosis in alcoholics can be associated with thiamine deficiency, hyponatraemia, and neurological disorders.

    Observation:

    • Three alcoholic patients with severe lactic acidosis were treated with thiamine.
    • One patient had shoshin beriberi, another presented with hyponatraemia and rhabdomyolysis, and the third had Korsakoff's syndrome.

    Findings:

    • Thiamine alone corrected lactic acidosis within 2-4 hours in all three patients.
    • Correction of lactic acidosis occurred independently of haemodynamic or neurological improvements.
    • Thiamine was effective even when prior symptomatic treatments for shock and lactic acidosis had failed.

    Implications:

    • Thiamine should be considered a primary treatment for severe lactic acidosis in alcoholic patients.
    • The findings suggest thiamine's potential efficacy in other forms of lactic acidosis.
    • Early thiamine intervention may prevent severe complications associated with alcoholic lactic acidosis.
