Insights
Terbinafine resistance in children with T. rubrum dermatophytosis is a growing concern. This case highlights a rare instance of terbinafine resistance in a child with congenital ichthyosiform erythroderma.
Area of Science:
- Medical Mycology
- Pediatric Dermatology
- Antimicrobial Resistance
Background:
- Dermatophytosis is a common fungal infection in children.
- Trichophyton rubrum is a frequent cause of pediatric dermatophytosis.
- Terbinafine is the standard first-line treatment for T. rubrum infections.
Observation:
- A child with recurrent T. rubrum dermatophytosis and congenital ichthyosiform erythroderma was treated.
- The patient exhibited resistance to both topical and systemic terbinafine therapy.
- This represents the first reported case of terbinafine resistance in this specific pediatric population.
Findings:
- Recurrent T. rubrum infections can develop resistance to terbinafine.
- Congenital ichthyosiform erythroderma may be associated with treatment challenges in dermatophytosis.
- Standard antifungal treatments may not be effective in all pediatric cases.
Implications:
- The emergence of terbinafine resistance necessitates alternative treatment strategies for pediatric dermatophytosis.
- Further research is needed to understand the mechanisms of terbinafine resistance in T. rubrum.
- Clinicians should consider alternative antifungal agents for refractory cases, especially in patients with underlying skin conditions.
Abstract:
Dermatophytosis in children caused by Trichophyton rubrum is preferably treated with topical or systemic terbinafine. We report the first case of terbinafine resistance in a child with recurrent T. rubrum dermatophytosis and congenital ichthyosiform erythroderma.
