Summary

Aortic valve or root repair in patients 75 years or older shows low early mortality and morbidity. Long-term survival is comparable to the general population, making it a viable option for selected elderly patients.

Purpose of the Study:

  • To evaluate operative techniques and long-term outcomes of aortic valve or root repair in patients aged 75 years and older.
  • To assess the safety and efficacy of reconstructive aortic surgery in a geriatric population.

Main Methods:

  • Retrospective analysis of 100 patients (≥75 years) who underwent aortic valve or root repair between 2002 and 2016.
  • Included assessment of surgical techniques (root repair, cusp repair, or combined) and follow-up for mortality and reoperation.

Main Results:

  • Early mortality (30-day) and permanent neurologic deficit rates were both 2%.
  • Long-term follow-up (427 patient-years) showed only one reoperation for aortic insufficiency.
  • Estimated 5- and 8-year survival rates were 76.4% and 71.3%, respectively, comparable to age-matched populations.

Conclusions:

  • Reconstructive aortic valve surgery is a safe and effective option for selected elderly patients.
  • Experienced surgical teams can achieve low mortality and morbidity, with long-term survival comparable to the general population.
