Summary

Aphasic status epilepticus (ASE) occurred in a patient with severe kidney disease due to missed dialysis. Prompt treatment with valproic acid and uremia correction resolved the condition.

Background:

  • Aphasic status epilepticus (ASE) is a rare neurological disorder.
  • Chronic kidney disease (CKD) can lead to complex medical issues.

Observation:

  • A 76-year-old woman with CKD developed ASE after neglecting peritoneal dialysis.
  • Clinical presentation included recurrent aphasia without other cognitive deficits.
  • Electroencephalography revealed left frontotemporal ictal discharges.

Findings:

  • Magnetic resonance imaging did not show a causative lesion.
  • Intravenous valproic acid and correction of uremia led to ASE resolution.
  • This is the first reported case of ASE associated with acute uremic crisis.

Implications:

  • Highlights the potential neurological complications of untreated uremia.
  • Suggests ASE as a possible manifestation of severe metabolic disturbances.
