Background:

  • Thrombotic thrombocytopenic purpura (TTP) is a rare, life-threatening thrombotic microangiopathy.
  • Pregnancy is associated with several microangiopathies, including TTP, HELLP syndrome, and pre-eclampsia.

Observation:

  • A 21-year-old female presented with hemolytic anemia, thrombocytopenia, and placental infarcts.
  • A negative Coomb's test raised suspicion for thrombotic microangiopathy.

Findings:

  • The patient was diagnosed with TTP and received emergency plasma exchange.
  • Immediate recovery was observed following plasma exchange treatment.

Implications:

  • Early initiation of plasma exchange is crucial for TTP management, even with pending workup.
  • Differentiating TTP from other pregnancy-related microangiopathies is essential for appropriate treatment decisions.
