Purpose of the Study:

  • To develop and validate the Atopic Dermatitis Symptom Score (ADSS) for smartphone-based self-assessment.
  • To evaluate the reliability and validity of the ADSS in children and adolescents with AD.
  • To establish the screening accuracy of ADSS for severe AD.

Main Methods:

  • 307 children and adolescents with AD enrolled.
  • Parents/caregivers recorded daily symptoms (itching, sleep, erythema, dryness, oozing, edema) via a smartphone app.
  • Statistical analyses included test-retest reliability, concurrent validity (vs. SCORAD), MCID, responsiveness, and screening accuracy using ROC analysis.

Main Results:

  • Good test-retest reliability (ICC=0.82).
  • Significant association between ADSS and SCORAD (r=0.64), indicating concurrent validity and responsiveness (r=0.56).
  • Optimal ADSS cutoff of 7.0 showed high sensitivity (88.4%) and specificity (78.6%) for severe AD prediction.

Conclusions:

  • The ADSS is a reliable and valid tool for self-assessment of atopic dermatitis symptoms in children.
  • The smartphone application facilitates easy daily recording and monitoring of AD symptoms.
  • ADSS demonstrates potential as a screening tool for identifying severe cases of childhood AD.
