Background:

  • Juvenile nasopharyngeal angiofibroma (JNA) is the most common benign nasopharyngeal tumor, primarily affecting adolescent males.
  • JNAs present with unilateral nasal obstruction and recurrent epistaxis, locally invading surrounding structures.
  • Surgical excision is the standard treatment for JNA.

Observation:

  • A 32-year-old male presented with an incidental finding on cranial MR imaging.
  • The imaging revealed a localized vascular mass lesion within the right sphenoid sinus.
  • No clinical signs, symptoms, or endoscopic evidence of disease were present.

Findings:

  • The lesion was identified as an angiofibroma.
  • This represents a rare presentation of angiofibroma confined exclusively to the sphenoid sinus.
  • The patient was asymptomatic, with the diagnosis made incidentally via imaging.

Implications:

  • This case expands the known anatomical distribution of angiofibromas.
  • It highlights the importance of advanced imaging in detecting rare presentations of common tumors.
  • Further investigation may be warranted for isolated sphenoid sinus angiofibromas in adults.
