Background:

  • The Nanostim™ leadless pacemaker (LP) is a novel device for patients requiring VVIR pacing.
  • Post-market surveillance is crucial for evaluating the real-world safety and performance of new medical technologies.

Purpose of the Study:

  • To confirm the short-term safety of the Nanostim™ leadless pacemaker (LP) in a prospective, multicentre, post-market study.
  • To report the primary safety results of the LEADLESS Observational Study, assessing freedom from serious adverse device effects (SADEs) at 6 months.

Main Methods:

  • A prospective, single-arm, multicentre study enrolled 470 subjects indicated for VVIR pacing.
  • Subjects were followed for adverse events at 90 days, 180 days, and 6-month intervals post-implantation.
  • The primary safety endpoint evaluated freedom from SADEs at 6 months in 300 subjects enrolled after a protocol pause and retraining.

Main Results:

  • The primary safety endpoint was met, with 94.6% freedom from SADEs at 6 months in the post-pause cohort, demonstrating non-inferiority to the performance goal (86%).
  • Overall, 6.6% of 470 subjects experienced SADEs, with cardiac perforation (1.3%) and vascular complications (1.3%) being most frequent.
  • Following protocol changes after the study pause, a statistically significant difference in final LP location and fewer repositioning attempts were observed, with a decreasing trend in cardiac perforation and device dislodgement rates.

Conclusions:

  • The Nanostim™ leadless pacemaker (LP) met its primary 6-month safety endpoint.
  • Protocol modifications and enhanced training following a temporary pause significantly reduced the rates of cardiac perforation and device dislodgement, improving device safety.
