    Background:

    • Cauda equina syndrome (CES) is a surgical emergency.
    • Central disc lesions are a common cause of CES.
    • Understanding different presentations of CES may impact surgical timing and outcomes.

    Purpose of the Study:

    • To identify distinct modes of presentation for CES secondary to central disc lesions.
    • To evaluate the impact of presentation mode and surgical timing on patient outcomes, particularly bladder and motor function.

    Main Methods:

    • Retrospective chart review of 31 patients with CES due to central disc lesions.
    • Analysis of patient presentation, symptoms, surgical intervention, and functional recovery.
    • Categorization of presentations into acute and slower onset modes.

    Main Results:

    • Two modes of presentation identified: acute (10 patients) and slower onset (21 patients).
    • Acute presentation associated with more severe symptoms and poorer bladder function prognosis.
    • Excellent motor function recovery (27/30 operated patients); bladder function remained the most affected postoperatively.
    • No correlation found between time to surgical decompression and functional recovery.

    Conclusions:

    • CES from central disc lesions can present acutely or gradually, influencing prognosis, especially for bladder function.
    • While early surgical decompression is recommended, the strict <6-hour window previously suggested may not be essential for recovery.
    • Focus on surgical decompression remains crucial, but the precise timing requires individualized assessment based on presentation.
