Young women using family planning services have high rates of chlamydia infection. Receiving condoms was associated with higher chlamydia positivity, regardless of the primary contraceptive method used.
Area of Science:
- Reproductive Health
- Infectious Disease Epidemiology
- Public Health
Background:
- Adolescent girls and young women face high rates of sexually transmitted infections (STIs).
- Current contraceptive methods offer limited protection against STIs.
- Few studies have analyzed chlamydial infection rates based on contraceptive method use.
Purpose of the Study:
- To examine the prevalence of chlamydial infection among female family planning clients.
- To compare chlamydia positivity rates across different contraceptive method tiers.
- To assess the association between condom provision and chlamydia positivity.
Main Methods:
- Cross-sectional analysis of claims data from a California family planning program (July 2012-June 2013).
- Linked contraceptive method data with chlamydia laboratory test results for 74,636 female clients aged 15-29.
- Classified contraceptive methods into tiers: tier 1 (high-efficacy long-acting/permanent), tier 2 (shorter-acting hormonal), and tier 3 (other methods).
- Used log-binomial models to estimate adjusted prevalence ratios.
Main Results:
- Overall chlamydia positivity was 5.1% among the study population.
- Chlamydia positivity was highest in tier 2 users (5.3%) compared to tier 1 (4.5%) and tier 3 (4.9%), though adjusted analyses showed no significant differences by tier.
- Clients who received condoms had significantly higher chlamydia positivity (6.3% vs. 4.3%) and were 1.32 times more likely to test positive.
Conclusions:
- High chlamydia positivity exists among young women utilizing family planning services, irrespective of their primary contraceptive method.
- The findings highlight an unmet need for integrated prevention strategies.
- Development of Multipurpose Prevention Technologies offering dual protection against pregnancy and STIs is recommended.
