    Background:

    • Carotid endarterectomy is a common procedure to prevent stroke.
    • Pseudoaneurysms are rare but serious complications following endarterectomy.
    • Infection of vascular grafts is a significant concern.

    Observation:

    • A patient developed a recurrent infected pseudoaneurysm seven years after carotid endarterectomy with a Dacron patch.
    • The initial pseudoaneurysm was treated with resection and patch angioplasty.
    • The recurrence was attributed to infection of the prosthetic Dacron patch.

    Findings:

    • Resection of the infected pseudoaneurysm and reconstruction with an autologous saphenous vein patch resulted in a cure.
    • Infection is an uncommon cause of pseudoaneurysm after carotid endarterectomy, accounting for only 14% of reported cases.
    • Prosthetic patch materials may increase the risk of infection and pseudoaneurysm formation.

    Implications:

    • Avoiding prosthetic materials in carotid endarterectomy may reduce the incidence of infected pseudoaneurysms.
    • Management of infected pseudoaneurysms requires precise diagnosis, careful surgical planning, and complete debridement.
    • Autologous vein grafts are a reliable option for reconstruction in cases of infected pseudoaneurysm after carotid endarterectomy.
