α,β-Unsaturated carbonyl compounds with two electrophilic sites, the carbonyl carbon, and the β carbon, are susceptible to nucleophilic attack via two modes: conjugate or 1,4-addition and direct or 1,2-addition.

Background:

  • Keratoconus management often involves intrastromal corneal ring segments (ICRS) like INTACS.
  • Sequential implantation of ICRS is an evolving strategy for advanced keratoconus.

Observation:

  • KeraRing implantation was performed without removing prior INTACS segments in 5 eyes.
  • Manual tunnel creation was used for KeraRing insertion.
  • Visual acuity, refractive, and keratometric data were collected pre- and post-implantation.

Findings:

  • Four out of five eyes showed improved uncorrected and corrected distance visual acuity (UDVA and CDVA).
  • Significant reductions in mean keratometric values were observed.
  • Higher-order aberrations improved in all cases, with one patient reporting glare.

Implications:

  • Sequential KeraRing implantation following INTACS can enhance visual and refractive outcomes in selected keratoconus patients.
  • This technique may offer a solution for cases not fully corrected by initial ICRS.
  • Careful patient selection is crucial for optimizing results and minimizing side effects like glare.
