Summary

Robotic subxiphoid thymectomy (RST) offers improved visualization and easier identification of phrenic nerves compared to lateral thoracic approaches for myasthenia gravis and thymomas. This minimally invasive technique is suitable for complex procedures and may become the standard of care.

Area of Science:

  • Thoracic surgery
  • Surgical oncology
  • Minimally invasive procedures

Background:

  • Lateral thoracic approaches for thymectomy in myasthenia gravis and thymomas can limit operative field visualization, particularly in the neck.
  • Identifying the contralateral phrenic nerve can be challenging with unilateral thoracic approaches.

Purpose of the Study:

  • To present the techniques for robotic subxiphoid thymectomy (RST).
  • To highlight the advantages of RST in improving surgical field visualization and anatomical landmark identification.

Main Methods:

  • RST involves inserting the camera via a subxiphoid incision and robotic instruments through bilateral intercostal spaces.
  • A subxiphoid single-port thymectomy is used for procedures without suturing.
  • RST is selected for thymectomies requiring suturing, such as pericardial patch closure.

Main Results:

  • The subxiphoid approach provides a surgical field comparable to median sternotomy.
  • RST facilitates easier identification of bilateral phrenic nerves and enhances neck area visualization.
  • Robotic assistance improves operability for complex thymectomy procedures.

Conclusions:

  • Robotic subxiphoid thymectomy offers superior visualization and anatomical identification compared to traditional lateral thoracic approaches.
  • RST is a versatile and effective technique for thymectomy, adaptable to varying procedural complexities.
  • The study suggests that robot-assisted thymectomy, particularly the RST approach, may become the standard surgical method in the future.
