Background:

  • Contact Heat Evoked Potentials (CHEPs) are used to assess small fiber neuropathy.
  • Reliability of CHEPs in lower extremities requires further investigation.

Purpose of the Study:

  • To evaluate the test-retest reliability of CHEPs in the lower extremities.
  • To compare two stimulation protocols: normal and increased baseline temperature.

Main Methods:

  • 32 healthy subjects participated, with 22 retested.
  • CHEPs were recorded from L2, L5, and S2 dermatomes.
  • Test-retest reliability was analyzed using different stimulation protocols.

Main Results:

  • Increased baseline temperature improved CHEPs acquisition, especially at distal sites (L5, S2).
  • CHEP latency (N2 potential) showed the highest reliability.
  • CHEP amplitudes demonstrated fair to excellent reliability.

Conclusions:

  • Increasing thermode baseline temperature enhances CHEPs acquisition from lower extremities.
  • CHEPs are a reliable diagnostic tool for evaluating small fiber integrity.
