Summary

Polytrauma patients with a Glasgow Coma Scale (GCS) of 3 have a poor prognosis, but aggressive resuscitation can improve survival. Young patients responding well to resuscitation show better functional recovery.

Background:

  • Polytrauma patients with a Glasgow Coma Scale (GCS) of 3 represent a critical subgroup with significant mortality.
  • Understanding prognostic factors is crucial for optimizing emergency department (ED) management and resource allocation.

Purpose of the Study:

  • To clarify the prognosis of polytrauma patients presenting to the ED with a GCS of 3.
  • To identify factors associated with survival and functional recovery in this patient population.

Main Methods:

  • Retrospective review of a trauma registry from January 2011 to December 2015.
  • Analysis of 145 polytrauma patients presenting with a GCS of 3.
  • Comparison of surviving and non-surviving patients to determine prognostic factors for survival and discharge GCS > 13.

Main Results:

  • Mortality rate was high (82.1%), with 119 of 145 patients not surviving.
  • Independent predictors of survival included the absence of bilateral dilated and fixed pupils and a GCS > 3 after resuscitation.
  • Independent predictors for a discharge GCS > 13 were age under 40 and a GCS > 3 after resuscitation.

Conclusions:

  • Aggressive resuscitation benefits polytrauma patients with a GCS of 3, particularly those showing a rapid response.
  • Younger patients with severe traumatic brain injury (GCS of 3) have a higher likelihood of functional recovery following ED resuscitation.
