Comparison between abdominal and hysteroscopic metroplasty.

    Summary

Hysteroscopic metroplasty for septate uteri resulted in better reproductive outcomes and fewer complications compared to the Tompkins procedure. This suggests hysteroscopic metroplasty is a more effective surgical option for uterine septum correction.

    Area of Science:

    • Reproductive Medicine
    • Minimally Invasive Surgery
    • Gynecologic Surgery

    Background:

    • Septate uterus is a common congenital uterine anomaly.
    • It is associated with infertility, recurrent pregnancy loss, and preterm labor.
    • Surgical correction is often necessary to improve reproductive outcomes.

    Purpose of the Study:

    • To compare the effectiveness of hysteroscopic metroplasty versus Tompkins metroplasty for the treatment of septate uterus.
    • To evaluate the reproductive outcomes and complication rates associated with each surgical approach.

    Main Methods:

    • A retrospective study of 33 patients diagnosed with septate uterus.
    • 14 patients underwent Tompkins metroplasty (Group 1) and 19 underwent hysteroscopic metroplasty (Group 2).
    • Post-operative hysterosalpingography and pregnancy outcomes were analyzed.

    Main Results:

    • Hysteroscopic metroplasty resulted in a higher rate of normal uterine cavities (88% vs. 72%) and fewer intrauterine defects (0% vs. 14%).
    • Pregnancy rates were higher in the hysteroscopic group (84% vs. 71%), with a higher proportion of term vaginal deliveries (87% vs. 70%).
    • Hysteroscopic metroplasty was associated with shorter operative time, less blood loss, and shorter hospital stays.

    Conclusions:

    • Hysteroscopic metroplasty is a safe and effective procedure for correcting septate uterus.
    • It offers superior anatomical and reproductive outcomes compared to Tompkins metroplasty.
    • Hysteroscopic metroplasty is recommended due to lower morbidity and better patient outcomes.
