Background:

  • Pleomorphic leiomyosarcoma (PLMS) and dedifferentiated leiomyosarcoma (DLMS) are rare soft tissue tumors.
  • Distinguishing these entities from other high-grade sarcomas is critical for appropriate management.

Purpose of the Study:

  • To investigate the clinicopathologic features, differential diagnosis, and biological behavior of PLMS and DLMS.
  • To establish diagnostic criteria and understand the prognostic implications of these leiomyosarcoma subtypes.

Main Methods:

  • Retrospective review of 49 cases of PLMS and DLMS.
  • Analysis of clinical findings, histopathologic features, and immunophenotype (myogenic markers, Ki-67).
  • Follow-up data was collected to assess survival and disease progression.

  • Tumors showed a combination of typical leiomyosarcoma (LMS) and high-grade pleomorphic/undifferentiated sarcoma areas.

  • High rates of recurrence (31.6%), metastasis (28.9%), and disease-related death (28.9%) were observed, with median disease-free and overall survival of 6 and 10 months, respectively.

Conclusions:

  • Accurate diagnosis of PLMS and DLMS requires identifying areas of typical LMS.
  • PLMS and DLMS exhibit aggressive behavior and a poorer prognosis compared to conventional LMS.
  • These findings underscore the importance of precise histopathologic evaluation and immunohistochemistry for diagnosing and managing these challenging sarcomas.
