Insights

Cryptic Aspergillus species, like those in the Aspergillus viridinutans complex, cause invasive aspergillosis and often resist antifungals. This study evaluated their antifungal susceptibility, finding resistance to voriconazole and itraconazole but susceptibility to posaconazole and echinocandins.

Area of Science:

  • Medical Mycology
  • Antimicrobial Resistance
  • Clinical Microbiology

Background:

  • Cryptic species within the Aspergillus genus, particularly the Aspergillus viridinutans species complex, are increasingly recognized as significant pathogens causing invasive aspergillosis.
  • These cryptic species often exhibit intrinsic resistance to commonly used antifungal agents, posing a therapeutic challenge.

Purpose of the Study:

  • To evaluate the in vitro antifungal susceptibilities of environmental and clinical isolates belonging to the Aspergillus viridinutans species complex.
  • To compare the performance of the European Committee on Antimicrobial Susceptibility Testing (EUCAST) reference method with Sensititre YeastOne (YO) panels for determining these susceptibilities.

Main Methods:

  • DNA sequencing of the calmodulin gene was used to identify 90 isolates from the Aspergillus viridinutans species complex.
  • Antifungal susceptibility testing was performed using the EUCAST method against seven agents: voriconazole, posaconazole, itraconazole, amphotericin B, anidulafungin, micafungin, and caspofungin.
  • Results from the EUCAST method were compared with those obtained using Sensititre YeastOne (YO) panels read at 48 and 72 hours.

Main Results:

  • The majority of isolates showed elevated minimum inhibitory concentrations (MICs) for voriconazole (geometric mean [GM] MIC, 4.46 mg/liter) and itraconazole (GM MIC, 9.85 mg/liter), with variable susceptibility to amphotericin B (GM MIC, 2.5 mg/liter).
  • Posaconazole and echinocandins demonstrated low MICs and minimum effective concentrations, respectively.
  • Overall agreement between EUCAST and YO panels was 67% at 48 hours and 82% at 72 hours, with high agreement for amphotericin B, voriconazole, and posaconazole, but very low agreement for itraconazole.

Conclusions:

  • The Aspergillus viridinutans species complex frequently displays resistance to voriconazole and itraconazole, highlighting the need for accurate species identification and susceptibility testing.
  • Posaconazole and echinocandins appear to be more reliable therapeutic options against these cryptic Aspergillus species.
  • The EUCAST method provides a reliable basis for susceptibility testing, although agreement with YO panels varies significantly by antifungal agent and species, particularly for itraconazole.

Renal Drug Clearance: Comparison Between Renal Excretion Methods

Renal clearance is a critical parameter encompassing kidney filtration, secretion, and reabsorption processes. It is calculated using a specific equation to determine the rate at which the kidneys clear a drug.
Renal clearance is often associated with the renal glomerular filtration rate (GFR), which represents the rate at which plasma is filtered through the glomeruli in the kidney. When drug reabsorption is minimal and there is no active secretion, renal clearance is closely related to the...

The Sense of Self: Reflected Self-Appraisal and Social Comparison

According to Charles Cooley, we base our image on what we think other people see (Cooley 1902). We imagine how we must appear to others, then react to this speculation. We don certain clothes, prepare our hair in a particular manner, wear makeup, use cologne, and the like—all with the notion that our presentation of ourselves is going to affect how others perceive us. We expect a certain reaction, and, if lucky, we get the one we desire and feel good about it. But more than that, Cooley...

Magnetic Susceptibility and Permeability01:31

Magnetic Susceptibility and Permeability

In linear magnetic materials, like paramagnets and diamagnets, magnetization is proportional to the magnetic field intensity. The constant of proportionality, a dimensionless number, is called magnetic susceptibility. The value of the susceptibility depends on the type of material.
When diamagnetic materials are placed under an external magnetic field, the moments opposite to the field are induced. Hence, the susceptibility for diamagnets has a minimal negative value of 10-5–10-6. Since...

Protein Complex Assembly02:41

Protein Complex Assembly

Proteins can form homomeric complexes with another unit of the same protein or heteromeric complexes with different types.  Most protein complexes self-assemble spontaneously via ordered pathways, while some proteins need assembly factors that guide their proper assembly. Despite the crowded intracellular environment, proteins usually interact with their correct partners and form functional complexes.
Many viruses self-assemble into a fully functional unit using the infected host cell to...

Susceptibility, Permittivity and Dielectric Constant01:26

Susceptibility, Permittivity and Dielectric Constant

When placed in an external electric field, a dielectric material gets polarized. The charge density in the dielectric material is given by the sum of the bound and free charge densities, while the total charge density can also be written in terms of the total electric field. The bound charge density can be measured in terms of polarization, leading to the relationship between electric displacement and polarization.

Multiple Comparison Tests01:13

Multiple Comparison Tests

Multiple comparison test, abbreviated as MCT, is a post hoc analysis generally performed after comparing multiple samples with one or more tests. An MCT will help identify a significantly different sample among multiple samples or a factor among multiple factors.
It would be easy to compare two samples using a significance alpha level of 0.05. In other words, there is only one sample pair to be compared. However, it would be difficult to identify a significantly different sample if the number...
