Ventral mesh rectopexy (VMR) using biological mesh significantly improves posterior pelvic compartment symptoms and patient satisfaction. Outcomes were similar for both laparoscopic (LVMR) and robotic (RVMR) approaches, with low complication rates.
Area of Science:
- Minimally invasive surgery
- Pelvic floor disorders
- Surgical outcomes
Background:
- Ventral mesh rectopexy (VMR) is a nerve-sparing procedure for symptomatic posterior pelvic compartment issues.
- Biological mesh is utilized in VMR to address morphological changes.
Purpose of the Study:
- To evaluate the short-term functional outcomes and patient satisfaction following laparoscopic (LVMR) and robotic (RVMR) ventral mesh rectopexy (VMR) using biological mesh.
- To compare the efficacy and safety of LVMR versus RVMR.
Main Methods:
- Analysis of data from 123 patients undergoing LVMR or RVMR between August 2012 and January 2017.
- Assessment of patient demographics, surgical findings, and validated scoring systems including Cleveland Clinic Constipation Score (CCCS), Obstructed Defecation Score (ODS), and Cleveland Clinic Incontinence Score (CCIS).
- Patient satisfaction was measured using a visual analog scale (0-10).
Main Results:
- Statistically significant improvements were observed in CCCS, CCIS, and ODS at 6 and 12 months (p < 0.001).
- Excellent patient satisfaction was reported at 6 and 12 months (8.2/10 and 8.3/10, respectively).
- The overall complication rate was 14% (2% major), with no mesh-related complications. Re-intervention rate was 3%. Outcomes were comparable between LVMR and RVMR.
Conclusions:
- Laparoscopic and robotic ventral mesh rectopexy with biological mesh are safe and effective treatments for posterior pelvic compartment disorders.
- Both procedures lead to significant symptom reduction and high patient satisfaction.
- LVMR and RVMR demonstrate similar short-term functional outcomes and safety profiles.
