Summary

Diffuse astrocytoma, IDH-wildtype, may not exist as a distinct entity. Most low-grade appearing IDH-wildtype tumors show molecular and clinical signs of high-grade glioma, suggesting early glioblastoma.

Background:

  • The existence and characteristics of diffuse astrocytoma, IDH-wildtype, are poorly understood.
  • Accurate classification is crucial for diagnosis and treatment of adult diffuse astrocytomas.

Purpose of the Study:

  • To investigate the histological, molecular, and clinical features of diffuse astrocytoma, IDH-wildtype.
  • To clarify the diagnostic and biological status of IDH-wildtype diffuse astrocytomas in adults.

Main Methods:

  • Analysis of 212 adult diffuse astrocytomas (WHO grade II) using IDH1/IDH2 sequencing and immunohistochemistry.
  • Neuroimaging review, DNA methylation profiling (Infinium HumanMethylation450k BeadChip), and copy number analysis.
  • Classification based on molecular and histological data.

Main Results:

  • Only 25/212 tumors lacked IDH1/IDH2 mutations and contrast enhancement.
  • Of these, 17 showed molecular or genetic features of glioblastoma, IDH-wildtype, despite low-grade histology.
  • 10/17 patients with these tumors experienced early malignant progression.
  • Other molecular classifications included diffuse midline glioma, IDH-mutant astrocytoma, pilocytic astrocytoma, and normal brain tissue.

Conclusions:

  • No distinct entity of diffuse astrocytoma, IDH-wildtype, was identified in this cohort.
  • IDH-wildtype tumors with low-grade features often possess molecular and clinical characteristics of high-grade glioma.
  • These findings suggest that such tumors may represent an early stage of primary glioblastoma, impacting classification and diagnosis.
