[Osteonecrosis in acne conglobata].

M Knüchel, C Luderschmidt

    Zeitschrift Fur Hautkrankheiten
    |August 1, 1986

    Summary

A severe acne complication, acne fulminans, led to osteomyelitis and osteonecrosis of the clavicle in a teenage patient. This case highlights the potential for deep bone infection originating from severe acne lesions.

    Area of Science:

    • Dermatology
    • Orthopedic Surgery
    • Infectious Diseases

    Background:

    • Acne conglobata is a severe form of acne vulgaris.
    • Acne fulminans is an uncommon, acute presentation of acne conglobata, often associated with systemic symptoms.
    • Bone infections are serious conditions requiring prompt diagnosis and treatment.

    Observation:

    • A 17-year-old male patient presented with acne conglobata.
    • The patient subsequently developed a rash consistent with acne fulminans.
    • Following the acne fulminans outbreak, osteomyelitis and osteonecrosis of the left clavicle were diagnosed.

    Findings:

    • The bone infection (osteomyelitis) and bone death (osteonecrosis) were directly linked to the acne fulminans.
    • Infection pathway identified: spread from a cystic acne nodule into the adjacent clavicular bone.
    • This case demonstrates a rare but severe complication of acne fulminans.

    Implications:

    • Highlights the importance of recognizing severe acne presentations and their potential systemic and invasive complications.
    • Suggests a need for vigilance in monitoring patients with severe acne for signs of secondary infections, including bone involvement.
    • Underscores the multidisciplinary approach required for managing complex cases involving dermatological and orthopedic conditions.
