Background:

Chronic recurrent multifocal osteomyelitis (CRMO) is a rare, noninfectious inflammatory bone disease.
It presents with multifocal bone lesions, swelling, and pain, often with unclear pathogenesis.
Radiological findings include lytic/sclerotic lesions on X-ray and bone marrow edema on STIR MRI.

Observation:

A 10-year-old girl presented with a year of right shoulder pain.
Diagnosis was confirmed through clinical, laboratory, radiological findings, and bone biopsy.
The patient's condition improved with anti-inflammatory treatment.

Findings:

CRMO diagnosis should be considered in children with recurrent bone pain.
Suspect CRMO with modest inflammatory markers and characteristic bone lesions on X-ray.
Common CRMO sites include long bone metaphyses, pelvis, and spine, but any bone can be affected.

Implications:

Early diagnosis of CRMO in children is vital to prevent long-term bone damage.
Differential diagnosis must exclude infections, bone tumors, and Langerhans Cell Histiocytosis (LCH).
Prompt anti-inflammatory treatment can significantly improve patient outcomes.
