Summary

This study prospectively tracked 600 Tunisian multiple sclerosis (MS) patients over 10 years. Findings reveal a two-stage disability progression, with a faster initial worsening in males and progressive-onset cases, and a more aggressive course overall compared to Western populations.

Background:

Purpose of the Study:

  • To prospectively evaluate the disability progression of multiple sclerosis (MS) in a Tunisian cohort.
  • To define and analyze the distinct phases of MS disability progression.

Main Methods:

  • A prospective analysis of 600 patients from the MS database at the Mongi Ben Hmida National Institute of Neurology, Tunis.
  • Kaplan-Meier method used to estimate median durations of two defined MS phases (onset to DSS 3, and DSS 3 to DSS 6) and median ages at specific Disability Status Scale (DSS) scores.
  • Patient data were prospectively recorded over a 10-year period.

Main Results:

  • Median age at DSS 3 was 48 years and at DSS 6 was 53 years.
  • Median time from onset to DSS 3 was 9 years, and to DSS 6 was 12 years.
  • Phase 2 (DSS 3 to DSS 6) had a median duration of 3 years. Males and progressive-onset MS patients showed faster worsening in phase 1. Phase 2 progression was independent of gender and initial MS phenotype.

Conclusions:

  • Multiple sclerosis disability progression in Tunisian patients follows a two-stage pattern.
  • The disease course in this cohort appears more aggressive compared to Western populations.
  • Understanding these distinct progression phases and demographic influences is crucial for managing MS in North Africa.
