Summary

This study validated near-infrared transillumination (NIRT) for detecting proximal caries, enhancing it with high-dynamic-range imaging (HDRI). While NIRT-HDRI showed improved visualization, it did not significantly enhance diagnostic accuracy compared to standard NIRT.

Background:

Purpose of the Study:

  • To develop an in vitro model for validating NIRT in proximal caries detection.
  • To enhance NIRT with HDRI and compare its efficacy against standard NIRT.
  • To use micro-computed tomography (µCT) as the reference standard for validation.

Main Methods:

  • 53 human teeth were examined using Diagnocam (DC) and NIRT with HDRI (NIRT-HDRI).
  • NIRT-HDRI involved capturing and merging an exposure series into a single HDR image.
  • Examiners classified lesion depth, with assessments validated by µCT.

Main Results:

  • Both DC and NIRT-HDRI demonstrated very good inter-rater reliability (wκ 0.90 and 0.96, respectively).
  • Overall agreement between methods was almost perfect (wκ 0.85).
  • NIRT-HDRI showed higher sensitivity for sound surfaces and enamel caries, and higher specificity for dentin caries, but HDRI did not significantly improve diagnostics.

Conclusions:

  • NIRT, with or without HDRI, is a reliable method for proximal caries detection.
  • HDRI improves image visualization by managing luminance levels but does not significantly enhance diagnostic accuracy.
  • Differentiating demineralized enamel from dentin lesions remains a challenge for NIRT, irrespective of HDRI.
