Frontal sinus mucoceles are slow-growing cysts that can cause orbital symptoms. Endoscopic marsupialization offers a safe and effective treatment for these benign lesions, avoiding complications.

- Mucoceles are benign, slow-growing, mucous-filled cystic lesions originating in the paranasal sinuses.
- These lesions can cause progressive bony wall distension and lead to compressive symptoms.

- A patient presented with a history of progressive unilateral proptosis.
- Computed tomography revealed a large frontal sinus mucocele with ipsilateral orbital extension.
Findings:
- The patient underwent successful endoscopic marsupialization for the frontal sinus mucocele.
- The procedure was completed without any significant complications.
Implications:
- Endoscopic marsupialization is a viable and safe surgical option for treating frontal sinus mucoceles with orbital extension.
- This minimally invasive approach can effectively resolve compressive symptoms and prevent further complications.
