Clinical subtypes in panic disorder: their descriptive and prospective validity.

R Buller, W Maier, O Benkert

    Journal of Affective Disorders
    |September 1, 1986
    PubMed
    Summary

Agoraphobia presence indicates more severe panic anxiety in patients. However, a history of depression, while not affecting anxiety severity, led to greater impairment and more depressive episodes over time.

    Area of Science:

    • Psychiatry
    • Clinical Psychology
    • Anxiety Disorders

    Background:

    • Panic disorder is often comorbid with agoraphobia and depression.
    • Understanding the distinct impact of these comorbidities is crucial for accurate diagnosis and treatment.
    • Previous research suggests varying trajectories for panic disorder with and without these associated conditions.

    Purpose of the Study:

    • To investigate the longitudinal impact of agoraphobia and depression on the severity and course of panic disorder.
    • To explore whether agoraphobia and depression influence the incidence of major depression in patients with panic attacks.
    • To propose potential subtypes for panic disorder classification based on the presence of agoraphobia or depression.

    Main Methods:

    • A cohort of 97 patients experiencing panic attacks was assessed.

    Related Experiment Videos

  • Patients were evaluated at baseline and followed up over one year.
  • The study examined the association between panic anxiety severity, agoraphobia, and depression history.
  • Main Results:

    • Agoraphobia was associated with more severe panic anxiety at baseline and during follow-up.
    • The presence of agoraphobia did not increase the incidence of major depression.
    • Patients with a history of depression (primary or secondary) showed similar anxiety severity but greater overall impairment and higher rates of subsequent depressive episodes.

    Conclusions:

    • Agoraphobia is a marker of more severe panic anxiety in patients with panic disorder.
    • Depression history in panic disorder patients is linked to increased functional impairment and a higher risk of recurrent depression.
    • Subtyping panic disorder based on the constant presence of agoraphobia or depression may improve future classification systems.
