Regional cerebral glucose utilization during hyperglycemia.

    Summary

    Hyperglycemia reduces brain blood flow. This study found that while acute high blood sugar doesn't change brain glucose use, chronic hyperglycemia does slightly lower it, but not enough to explain the blood flow drop.

    Background:

    • Previous studies show hyperglycemia decreases regional cerebral blood flow (rCBF).
    • The link between reduced rCBF and brain metabolism during hyperglycemia is not fully understood.

    Purpose of the Study:

    • To investigate if decreased rCBF during hyperglycemia is due to reduced cerebral metabolic rate.
    • To measure regional cerebral glucose utilization (rCMRgl) in rats under acute and chronic hyperglycemia.

    Main Methods:

    • Rats were subjected to acute hyperglycemia (glucose injection) and chronic hyperglycemia (streptozotocin treatment).
    • Regional cerebral glucose utilization (rCMRgl) was measured using [6-14C]glucose over 10 minutes.

    Main Results:

    • rCMRgl remained normal during acute hyperglycemia.
    • A 13% decrease in rCMRgl was observed after 3 weeks of chronic hyperglycemia.
    • The findings suggest that altered brain metabolism does not fully explain reduced rCBF in hyperglycemia.

    Conclusions:

    • Reduced rCBF during acute hyperglycemia is not caused by a decrease in cerebral metabolic rate.
    • While chronic hyperglycemia slightly reduces glucose utilization, ketone bodies may compensate, making it unlikely to be the sole cause of decreased rCBF.
