Background:

  • Eastern equine encephalitis (EEE) is a severe arboviral disease affecting the central nervous system.
  • Ophthalmic manifestations of EEE are poorly understood.

  • A septuagenarian male presented with symptoms consistent with viral meningoencephalitis.
  • Enzyme-linked immunosorbent assay (ELISA) indicated recent EEE virus infection.

Findings:

  • Histopathological analysis of the brain showed perivascular lymphohistiocytic cuffing, indicative of viral encephalitis.
  • Ocular examination revealed lymphohistiocytic infiltrate in the retina and optic nerve.
  • A reduction in the number of retinal ganglion cells was observed.

Implications:

  • This case provides the first description of ocular pathology in EEE.
  • Retinal inflammatory findings mirror central nervous system effects of EEE virus.
  • Findings are relevant in the context of arboviral infections with ophthalmic sequelae, such as Zika virus.
