Background:

Autoimmune endocrine disorders often coexist but typically present sequentially.
Simultaneous presentation of multiple endocrine disorders is uncommon.
This case examines the rare concurrent manifestation of Graves' disease and Addison's disease.

Observation:

A 67-year-old woman presented with generalized weakness, nausea, diarrhea, weight loss, tachycardia, and hypotension.
Initial diagnosis revealed Graves' thyrotoxicosis, with treatment initiated.
Despite treatment, the patient's condition rapidly deteriorated, prompting further investigation.

Findings:

Further tests confirmed a concomitant Addisonian crisis alongside Graves' thyrotoxicosis.
The patient received corticosteroids for Addisonian crisis, leading to a full recovery.
This represents a highly unusual simultaneous presentation of two distinct autoimmune endocrine conditions.

Implications:

Clinicians should consider the possibility of coexisting autoimmune endocrine disorders, even with seemingly distinct initial presentations.
Prompt and comprehensive diagnostic workups are essential when patients present with complex or deteriorating symptoms.
Integrated management strategies are vital for addressing multiple, simultaneously presenting autoimmune endocrine diseases effectively.
