    Background:

    • Endodontically treated teeth (ETT) often require restoration.
    • The use of endodontic posts to support restorations is a common clinical practice.
    • However, the clinical benefit of post insertion remains debated.

    Purpose of the Study:

    • To systematically review the impact of endodontic post insertion on the clinical performance of ETT.
    • To evaluate the success and survival rates of dental restorations with and without posts.

    Main Methods:

    • A systematic literature search was conducted in January 2017 across multiple databases (Medline, Opengrey, BBO, LILAC, IBECS).
    • Studies comparing restorations with and without endodontic posts were included.
    • Methodological quality was assessed using CASP and Cochrane checklist.

    Main Results:

    • Fourteen studies were included: 11 RCTs, 2 prospective, and 1 retrospective clinical trial.
    • While overall quality was good, seven studies had a high risk of bias.
    • Ten studies did not demonstrate a positive effect of post placement; a benefit may exist for no-cavity-wall situations.

    Conclusions:

    • There is insufficient clinical evidence to definitively support or refute the use of endodontic posts.
    • This applies to both direct and indirect restorations, even in cases lacking cavity walls.
    • Further high-quality research is warranted to clarify the role of posts in ETT restoration.
