    Background:

    • Diabetic diarrhea (DD) is a complication of diabetes mellitus.
    • The underlying mechanisms of DD are not fully understood.
    • Altered small intestinal permeability may contribute to gastrointestinal symptoms in diabetics.

    Purpose of the Study:

    • To investigate small intestinal permeability in patients with diabetic diarrhea.
    • To compare permeability markers between DD patients, uncomplicated diabetics, and healthy controls.

    Main Methods:

    • Assessed small intestinal permeability using oral mannitol and lactulose challenge tests.
    • Measured urinary recovery of mannitol and lactulose to calculate lactulose to mannitol excretion ratios (LMER).
    • Analyzed LMER in 12 DD patients, 10 uncomplicated diabetics, and 25 healthy controls.

    • Patients with DD exhibited significantly higher LMER compared to controls and uncomplicated diabetics.
    • Seven out of 12 DD patients had LMER values outside the normal range.

    • Small intestinal permeability is abnormal in a subset of patients experiencing diabetic diarrhea.
    • Elevated LMER suggests increased intestinal permeability may be a contributing factor to the pathogenesis of DD.
    • Further research is warranted to elucidate the precise role of intestinal permeability in DD.
