    Summary

    Chronic cigarette smoking primes platelets, making them more prone to aggregation upon activation. This study found increased platelet activation in vitro among smokers compared to nonsmokers.

    Background:

    • Cigarette smoking is a significant risk factor for cardiovascular diseases.
    • Platelets play a crucial role in hemostasis and thrombosis.

    Purpose of the Study:

    • To investigate the effect of chronic cigarette smoking on platelet function.
    • To compare platelet activation, aggregation, and release of specific proteins in smokers versus nonsmokers.
    • To identify potential correlations between smoking duration/intensity and platelet activity.

    Main Methods:

    • Evaluated platelet surface activation in vitro using Formvar film.
    • Assessed platelet aggregation in vivo and in vitro.
    • Measured in vivo release of platelet-specific proteins, including platelet factor 4 and beta-thromboglobulin.
    • Compared results between 20 healthy smokers and 23 healthy nonsmokers.

    Main Results:

    • Smokers exhibited significantly higher platelet aggregation on an activating surface in vitro compared to nonsmokers (80 vs. 43 aggregates, P < .01).
    • Older smokers (≥50 years) and those with longer smoking duration (≥20 years) showed enhanced in vitro platelet aggregation.
    • No significant differences were observed in circulating platelet aggregate ratio, platelet count, platelet factor 4, or plasma beta-thromboglobulin levels between groups.

    Conclusions:

    • Chronic cigarette smoking appears to prime platelets, increasing their propensity to aggregate when exposed to an in vitro stimulus.
    • Age and duration of smoking may further influence the pro-aggregatory effects of smoking on platelets.
    • While smoking primes platelets for aggregation, other measured parameters of platelet activation and release were not significantly different in this cohort.
