    Summary

    Magnetic resonance imaging (MRI) can assess bone marrow cellularity by measuring longitudinal relaxation time (T1). However, MRI cannot differentiate between various bone marrow disorders with similar cellularity levels.

    Background:

    • Bone marrow disorders, including malignant involvement and polycythemia vera, alter marrow cellularity.
    • Assessing bone marrow cellularity is crucial for diagnosis and treatment monitoring.

    Purpose of the Study:

    • To evaluate the utility of spin-echo magnetic resonance imaging (MRI) in assessing bone marrow cellularity.
    • To determine if MRI can differentiate between various bone marrow pathologies based on relaxation times.

    Main Methods:

    • Spin-echo MRI was performed at 0.35 T and 0.5 T on patients with malignant bone marrow involvement, polycythemia vera, reactive bone marrow, and healthy individuals.
    • Longitudinal relaxation time (T1) and transverse relaxation time (T2) were measured and correlated with histological assessment of bone marrow cellularity.

    • Increased T1 relaxation times were observed in malignant, polycythemia vera, and reactive bone marrow compared to normal marrow.

    • Neither T1 nor T2 could differentiate between various malignancies or between malignant and reactive marrow at high cellularity.

    Conclusions:

    • MRI is a valuable tool for assessing bone marrow cellularity.
    • MRI cannot reliably differentiate between different bone marrow disorders when cellularity is similar.
