Open Angle Glaucoma: Treatment01:27

Open Angle Glaucoma: Treatment

1.0K
In open-angle glaucoma, the iridocorneal angle remains open, but the trabecular meshwork becomes stiff, slowing down the outflow of aqueous humor. This causes a buildup of aqueous humor in the anterior chamber, leading to a sudden increase in intraocular pressure. The treatment for open-angle glaucoma focuses on reducing the elevated intraocular pressure by either decreasing the secretion of aqueous humor or increasing its outflow.
Drugs such as carbonic anhydrase inhibitors, α2- and...

Treatment Resistant Cancers02:56

Treatment Resistant Cancers

3.8K
Cancer is the second leading cause of death in the United States. A cancer cell is genetically unstable and hence can mutate faster. They can also modify their microenvironment and escape immune surveillance. The difficulties in treating cancer are further compounded by the emergence of rapid resistance to anticancer drugs. The most common ways to attain resistance in cancer cells include alteration in drug transport and metabolism, modification of drug target, elevated DNA damage response, or...

Parkinson's Disease: Treatment

1.2K
Neurodegenerative disorders, such as Parkinson's Disease (PD), involve the gradual and irreversible destruction of neurons in particular brain areas. These disorders exhibit standard features like proteinopathies, selective vulnerability of some neurons, and an interaction of intrinsic properties, genetics, and environmental influences in neural injury.
Parkinson's Disease is primarily a result of the loss of dopaminergic neurons in the substantia nigra pars compacta. The cornerstone of...

Alzheimer's Disease: Treatment

976
Alzheimer's Disease (AD), a neurodegenerative disorder, is pathologically identified by amyloid plaques and neurofibrillary tangles composed of tau protein. AD pharmacotherapy aims to manage cognitive symptoms, delay disease progression, and treat behavioral symptoms. The treatment is primarily symptomatic and palliative, with no definitive disease-modifying therapy available. Cholinesterase inhibitors, including donepezil (Aricept), rivastigmine (Exelon), and galantamine (Razadyne), are...

Kidney Transplant II: Surgical Procedure

444
Preoperative ManagementThe primary goals of preoperative management in kidney transplantation are to optimize the patient’s metabolic state and prepare them for surgery through diet adjustments, necessary dialysis, and tailored medical treatment. This phase also involves comprehensive infection screening and patient education about the surgical procedure and postoperative care to improve outcomes and adherence.Medical ManagementA comprehensive evaluation is required for both the living...

Anticholinesterase Agents: Poisoning and Treatment01:26

Anticholinesterase Agents: Poisoning and Treatment

1.7K
Anticholinesterases, also known as cholinesterase inhibitors, work by blocking the breakdown of acetylcholine, leading to its accumulation in the synaptic cleft. This accumulation indirectly enhances both muscarinic and nicotinic actions. These agents are classified as reversible or irreversible based on their mechanism of action.
Irreversible agents form a strong bond with the cholinesterase enzyme, making it inactive. The breakdown of the phosphorylated enzyme is...

    Summary

Surgical treatments for sleep disordered breathing (SDB) effectively reduce snoring and sleepiness. Osteotomies and uvulopalatopharyngoplasty/functional expansion pharyngoplasty (UPPP/FEP) showed the highest success rates in this study.

    Area of Science:

    • Otolaryngology
    • Sleep Medicine
    • Surgical Innovation

    Background:

    • Sleep disordered breathing (SDB) significantly impacts patient quality of life.
    • Surgical interventions are explored for managing snoring and SDB.
    • Comparative efficacy of various surgical techniques for SDB requires further investigation.

    Purpose of the Study:

    • To compare the effectiveness of different surgical treatments for SDB.
    • To evaluate outcomes in snoring and sleepiness following procedures like UPPP/FEP, osteotomy, septoplasty with turbinoplasty, and somnoplasty.

    Main Methods:

    • A cohort of 329 SDB patients underwent clinical evaluation, DISE, and PSG.
    • 84 patients received surgical treatment: UPPP/FEP, osteotomy, septoplasty/turbinoplasty, or somnoplasty.

  • Outcomes were assessed using validated questionnaires at 6 weeks and 6 months post-surgery, with success defined as a snoring VAS score ≤3.
  • Main Results:

    • All surgical techniques demonstrated significant and persistent reductions in snoring and sleepiness scores.
    • UPPP/FEP and osteotomy yielded greater snoring score reductions compared to somnoplasty and septoplasty.
    • Treatment success rates were 88% for UPPP/FEP, 92% for osteotomy, 61% for septoplasty, and 64% for somnoplasty.

    Conclusions:

    • Surgical treatments for SDB provide effective and lasting relief from snoring and sleepiness.
    • Osteotomies and UPPP/FEP procedures are associated with the highest success rates for SDB management.
