Background:

  • Early-onset scoliosis surgery carries a risk of surgical site infections (SSIs).
  • Vancomycin powder is safe for pediatric use, but its efficacy in preventing SSIs in early-onset scoliosis surgery was unproven.

Purpose of the Study:

  • To evaluate the effectiveness of intrawound vancomycin powder in reducing SSIs in patients undergoing surgery for early-onset scoliosis.

Main Methods:

  • A retrospective study compared SSIs in early-onset scoliosis patients between 2010-2016.
  • Patients were divided into two groups: those receiving intrawound vancomycin powder and those who did not.
  • Perioperative care was standardized, with the only difference being the introduction of vancomycin powder.

Main Results:

  • 191 procedures across 36 patients were analyzed.
  • The SSI rate was 13.8% in the control group (12 infections/87 procedures) versus 4.8% in the vancomycin group (5 infections/104 procedures).
  • The reduction in SSI rate with vancomycin powder was statistically significant (p = 0.038).

Conclusions:

  • Intrawound vancomycin powder significantly decreases the risk of SSIs in growing spine surgery for early-onset scoliosis.
  • The findings suggest vancomycin powder is effective even in patients with prior surgeries performed without its use.
