Background:

  • Comitant exotropia is a common condition requiring accurate measurement.
  • Traditional methods for measuring ocular deviation may be subjective and dependent on examiner skill.

Purpose of the Study:

  • To evaluate the efficacy of video-oculography (VOG) with alternate cover for quantitative analysis of comitant exotropia.
  • To establish the accuracy and reliability of VOG compared to the alternate prism cover test (APCT).

Main Methods:

  • Thirty-four subjects with comitant exotropia were assessed.
  • Ocular deviation was measured using both APCT by two ophthalmologists and VOG with alternate cover.
  • A rotating model eye was used to verify VOG accuracy, leading to a derived linear equation.

Main Results:

  • VOG demonstrated excellent positive correlation with the model eye rotation (R=1.000, p<0.001).
  • The derived linear equation was: rotation of model eye = 0.978 × value obtained using VOG for a model eye - 0.549.
  • VOG showed strong positive correlation with APCT, with no systematic bias identified via Bland-Altman plots. Inter-observer variability for APCT was ±4.63 PD, and test-retest variability for VOG was ±3.56 PD.

Conclusions:

  • Video-oculography (VOG) with alternate cover is a non-invasive and accurate method for quantifying ocular deviation in comitant exotropia.
  • VOG is independent of examiner proficiency, making it a valuable tool, especially when skilled personnel are unavailable.
