Background:

  • Extracorporeal membrane oxygenation (ECMO) is a life-support measure for severe respiratory and cardiac failure.
  • Myocardial calcification is a rare complication associated with critical illness and prolonged ECMO support.

Observation:

  • This case series describes three young patients experiencing extensive myocardial calcifications during prolonged ECMO.
  • Patients presented with severe acute respiratory distress syndrome, septic cardiomyopathy, postresuscitation cardiogenic shock, or septic shock.

Findings:

  • Myocardial calcifications were confirmed via echocardiography, CT scans, and cardiac biopsy.
  • Factors contributing to this complication include hemodynamic failure, acidosis, high-dose vasopressors, and renal failure.

Implications:

  • Clinicians must recognize this unusual and potentially severe ECMO complication.
  • Further research is needed to understand ECMO's role in the development of diffuse myocardial calcification.
