    Summary

    High-dose etodolac (500 mg b.i.d.) showed transient, reversible effects on renal function in normal subjects and patients with renal insufficiency. Chronic administration resulted in negligible cumulative impact on kidney function.

    Background:

    • Previous research indicated transient renal effects of etodolac at 200 mg twice daily.
    • The impact of higher etodolac doses on renal function requires further investigation.

    Purpose of the Study:

    • To evaluate the renal effects of a higher etodolac dose (500 mg b.i.d.).
    • To determine the time course and reversibility of renal effects with increased etodolac dosage.
    • To assess if higher doses prolong the transient renal effects observed previously.

    Main Methods:

    • Study involved 10 healthy volunteers and 9 patients with renal insufficiency.
    • Assessed effects of the initial 500 mg dose and 4 days of twice-daily administration.
    • Measured fractional excretions of sodium and chloride, urinary prostaglandin E2, inulin clearance, para-aminohippuric acid clearance, and urinary 6-keto-prostaglandin F1 alpha.

    Main Results:

    • Etodolac transiently decreased fractional excretions of sodium and chloride and urinary prostaglandin E2 in both groups.
    • In patients, etodolac also transiently reduced inulin and para-aminohippuric acid clearances and urinary 6-keto-prostaglandin F1 alpha.
    • No significant changes in renal function were observed with chronic administration in either group.

    Conclusions:

    • High-dose etodolac (500 mg b.i.d.) produces transient and fully reversible effects on renal function.
    • The cumulative effect of chronic, high-dose etodolac on renal function appears negligible.
    • These findings suggest a favorable renal safety profile for short-term high-dose etodolac use.
