Background:

  • Biliary complications are a significant concern after liver transplantation (LT).
  • Internal biliary stenting (IBS) is hypothesized to mitigate these complications, but supporting literature is limited.
  • End-to-end choledoco-choledocostomy is a common biliary reconstruction technique in LT.

Purpose of the Study:

  • To evaluate the impact of internal biliary stenting (IBS) on patient and biliary outcomes following liver transplantation (LT).
  • To assess the efficacy of IBS in reducing biliary anastomotic complications after end-to-end choledoco-choledocostomy.

Main Methods:

  • A retrospective study of 175 patients undergoing deceased donor LT with end-to-end choledoco-choledocostomy between 2009 and 2013.
  • Internal biliary stenting (IBS) was performed in 67 patients (38%) with small bile ducts (<5 mm).
  • Comparison of outcomes between patients with and without IBS, including analysis of risk factors for biliary complications.

Main Results:

  • Biliary complications occurred in 35% of patients and were significantly reduced by IBS (p=0.0003).
  • Anastomotic fistulas and stenoses were observed in 13% and 25% of patients, respectively.
  • Absence of IBS was associated with increased risk of both fistula (p=0.014) and stricture (p=0.003).

Conclusions:

  • Internal biliary stenting (IBS) insertion during liver transplantation effectively decreases anastomotic complications.
  • IBS is a valuable adjunct in reducing the incidence of biliary fistulas and stenoses post-LT.
  • The findings support the routine use of IBS in select LT cases to improve biliary outcomes.
