    Background:

    • Emergency manuals are vital safety tools in high-reliability organizations and are increasingly used in critical medical settings.
    • Existing anesthesiology emergency manuals primarily focus on intraoperative use.
    • Most efficacy data for emergency manuals involves experienced physicians, not novices.

    Purpose of the Study:

    • To assess the impact of an emergency manual on the performance of novice physicians (Post-Graduate Year 1 residents) during critical events in the Intensive Care Unit (ICU).

    Main Methods:

    • Forty-one first-year residents managed a simulated unstable bradycardia event in the ICU.
    • Participants were randomized into a control group (n=18) or an intervention group (n=23) with an emergency manual.
    • Performance was measured by the number of successful diagnostic and treatment interventions within a 10-minute simulation.

    Main Results:

    • The emergency manual group averaged 9.9 out of 12 interventions (83% success rate).
    • The control group averaged 7.1 out of 12 interventions (59% success rate).
    • This difference was statistically significant (p < 0.01).

    Conclusions:

    • Emergency manuals significantly enhance the management of critical events by novice physicians in simulated ICU settings.
    • The use of an emergency manual led to an average 23% improvement in intervention success for junior doctors.
