The Upf proteins that carry out nonsense-mediated decay (NMD) are found in all eukaryotic organisms, including humans. Each protein has an individual role, but they need to work in collaboration.   • This case highlights the importance of considering Mahaim fibers in unexplained wide complex tachycardias.
  • Successful radiofrequency ablation offers a curative option for Mahaim fiber-related arrhythmias.
  • Early diagnosis and intervention can prevent significant morbidity associated with recurrent tachycardias.
