Summary

A man developed sepsis and a swollen hand with a hemorrhagic bulla. Blood cultures identified Streptococcus pyogenes, leading to a diagnosis of bullous cellulitis and sepsis.

Area of Science:

Background:

  • Sepsis is a life-threatening organ dysfunction caused by a dysregulated host response to infection.
  • Cellulitis is a common bacterial skin infection that can lead to serious complications if untreated.

Observation:

  • A 53-year-old male presented with symptoms of sepsis and right hand swelling.
  • A large, hemorrhagic bulla rapidly formed on the dorsal aspect of the affected hand within 24 hours.

Findings:

  • Blood cultures were positive for Streptococcus pyogenes.
  • The clinical presentation and microbiological findings supported a diagnosis of bullous cellulitis.

Implications:

  • This case highlights the potential for Streptococcus pyogenes to cause severe bullous cellulitis and sepsis.
