Mycoplasma-induced pustulosis with perifollicular involvement

Mycoplasma pneumoniae-induced rash and mucositis (MIRM) can present as a rare pustular eruption. This case highlights a unique presentation and the first histopathologic findings of perifollicular neutrophilic infiltration in MIRM.

Background:
- Mycoplasma pneumoniae-induced rash and mucositis (MIRM) is a clinical syndrome involving mucosal and skin lesions.
- Commonly reported skin manifestations include vesiculobullous, targetoid, papular, macular, and morbilliform eruptions.
Observation:
- A 13-year-old boy presented with a pustular skin eruption.
- This presentation is exceptionally rare in MIRM, with only one prior report in the literature.
Findings:
- This is the second reported case of MIRM presenting with pustulosis.
- Histopathologic examination revealed perifollicular neutrophilic infiltration, a novel finding for MIRM.
Implications:
- This case expands the spectrum of cutaneous manifestations of MIRM.
- Understanding these histopathologic findings may aid in diagnosing and managing MIRM, particularly atypical presentations.
