Background:

  • Failed back surgery syndrome (FBSS) presents diagnostic challenges.
  • Differentiating epidural fibrosis (scar) from recurrent disk herniation is crucial for patient management.

Purpose of the Study:

  • To evaluate the effectiveness of magnetic resonance (MR) imaging with gadolinium-diethylenetriaminepentaacetic acid/dimeglumine (Gd-DTPA) in distinguishing postoperative epidural fibrosis from recurrent disk herniation in FBSS patients.

Main Methods:

  • Thirty FBSS patients underwent pre- and postcontrast MR imaging.
  • Gadolinium-enhanced T1-weighted spin-echo sequences were analyzed.
  • MR findings were correlated with surgical and pathologic data in 17 patients (19 disk levels).
