Insights

Long-term statin therapy significantly reduced cardiovascular events and mortality in patients with chronic kidney disease (CKD) and coronary artery disease (CAD). However, statins did not benefit CKD-only patients, suggesting tailored treatment approaches are needed.

Area of Science:

  • Cardiovascular Medicine
  • Nephrology
  • Pharmacology

Background:

  • Long-term statin therapy is known to reduce mortality and offer renoprotective effects in patients with chronic kidney disease (CKD).
  • Limited data exists from China regarding the efficacy of statin therapy in patients with both CKD and coronary artery disease (CAD).
  • Understanding statin's impact in CKD patients, with or without CAD, is crucial for optimizing cardiovascular and renal outcomes.

Purpose of the Study:

  • To compare the long-term effects of statin therapy on cardiovascular events, all-cause death, and cardiac death in 254 CKD patients.
  • To investigate the differential impact of statins in CKD patients stratified by the presence or absence of concomitant CAD.
  • To provide evidence-based insights into statin use for CKD patients in the Chinese population.

Main Methods:

  • A comparative study involving 254 CKD patients, followed for 4 years.
  • Patients were analyzed based on the presence (CKD+CAD) or absence (CKD-only) of coronary artery disease.
  • Outcomes assessed included cardiovascular events, all-cause mortality, and cardiac mortality.

Main Results:

  • Long-term statin therapy demonstrated significantly greater efficacy in CKD+CAD patients, reducing cardiovascular events by 22.2% (P=0.012).
  • In CKD+CAD patients, statins led to a substantial reduction in all-cause and cardiac deaths by approximately 28.1% (P<0.001).
  • Conversely, statin therapy showed no significant impact on the clinical outcomes of CKD-only patients.

Conclusions:

  • Long-term statin therapy markedly decreases cardiovascular events and mortality in CKD patients who also have CAD.
  • CKD patients without concomitant CAD exhibited a favorable prognosis and did not appear to benefit from statin treatment.
  • Findings suggest that statin therapy should be considered primarily for CKD patients with established CAD.

Coronary Artery Disease (CAD): An Overview with Scientific InsightsCoronary Artery Disease (CAD), often referred to as C-A-D, is a prevalent blood vessel disorder classified under the broader category of atherosclerosis. Atherosclerosis is a pathological process characterized by the hardening and narrowing of arteries due to the accumulation of atherosclerotic plaques. These plaques are composed of cholesterol, fatty substances, inflammatory cells, calcium, and fibrin, reducing blood flow to...

Coronary Artery Disease II: Pathophysiology

Coronary Artery Disease (CAD) originates from a series of events that impair the function of coronary arteries, the blood vessels responsible for delivering oxygen-rich blood to the heart muscle. The pathophysiology of CAD is closely linked to atherosclerosis, a chronic inflammatory and lipid-driven condition affecting the vascular endothelium.1. Endothelial DamageThe process begins with damage to the vascular endothelium, which serves as a protective barrier between the blood and the vessel...

Chronic Kidney Disease I: Introduction

Chronic Kidney Disease (CKD) arises when the kidneys progressively lose their ability to function, ultimately leading to end-stage renal disease. At this advanced stage, the kidneys can no longer filter waste or maintain essential body functions, requiring renal replacement therapy (RRT) through dialysis or a kidney transplant for survival.Early-stage chronic kidney disease and detection challengesIn CKD's early stages, symptoms often remain absent because healthy nephrons compensate for...

Interprofessional care for coronary artery disease includes pharmacological therapy and revascularization procedures.Pharmacological therapy for Coronary Artery Disease (CAD) aims to manage symptoms, prevent complications, and improve patient outcomes through various classes of medications:Antiplatelet Agents:Aspirin and Clopidogrel: These medications inhibit platelet aggregation, preventing blood clots, which is crucial for avoiding heart attacks and strokes. Chronic Kidney Disease II: Clinical Manifestations

Chronic Kidney Disease (CKD) progressively impairs multiple body systems due to the accumulation of uremic toxins, which disrupt cellular functions across various organs.Neurologic symptomsNeurologic symptoms often arise early in CKD, as uremic toxin buildup drives changes in cognitive and motor functions. Patients frequently experience fatigue, headache, confusion, difficulty concentrating, and, in severe cases, seizures. Peripheral neuropathy commonly manifests as burning sensations in the...

Coronary Artery Disease III: Clinical Manifestations

Coronary Artery Disease (CAD) is a primary health risk worldwide, leading to significant morbidity and mortality. The condition arises from the buildup of atherosclerotic plaques within the coronary arteries, resulting in diminished blood supply to the heart muscle. The clinical manifestations of CAD vary widely, from asymptomatic stages to severe, life-threatening conditions. Understanding these manifestations is crucial for early diagnosis and effective management.Angina Pectoris: The Warning...
