    Lipoprotein-X (Lp-X) shows high reliability in diagnosing cholestasis, outperforming traditional liver enzymes. Lp-X also offers earlier prognostic insights into cholestasis improvement.

    Context:

    • Lipoprotein-X (Lp-X) is a potential biomarker for cholestasis.
    • Evaluating Lp-X alongside established cholestasis enzymes is crucial for clinical utility.

    Purpose:

    • To assess the diagnostic and prognostic accuracy of Lipoprotein-X (Lp-X) in detecting and monitoring cholestasis.
    • To compare the reliability of Lp-X with standard cholestasis enzymes: alkaline phosphatase, leucine arylamidase, and gamma-glutamyltranspeptidase.
    • To investigate Lp-X's performance in differentiating intrahepatic and extrahepatic cholestasis.

    Summary:

    • Lipoprotein-X (Lp-X) demonstrated a satisfactory correlation (>90%) with cholestasis enzymes and proved more reliable than enzymes when compared to liver histology.
    • However, Lp-X lacked absolute specificity, showing false negatives in some histologically confirmed cholestasis cases and could not differentiate intra- from extrahepatic cholestasis.
    • In control groups, positive Lp-X results prompted further investigation for unsuspected hepatobiliary disease.

    Impact:

    • Lipoprotein-X (Lp-X) offers a more reliable diagnostic marker for cholestasis compared to traditional liver enzymes.
    • Early detection of cholestasis improvement is possible with Lp-X, preceding enzyme level normalization by 1-2 weeks.
    • Positive Lp-X results in non-hepatobiliary disease patients warrant further investigation for underlying liver conditions.
